Name Change

1.  On the top of the form mark Life Insurance Company of the Southwest

2. Complete contact information.

3. Complete the legal name change section. 

4. For 403bs & IRAs mark "Owner" 

5. For 457bs mark "Annuitant"

6.  You do not need to share your social security information again unless it has changed.

7. Be sure to check the correct reason for your name change. 

8. A photograph/copy of your updated driver's license will suffice as proof of your name change.

1.  Complete the date section.

2.  Sign with your new name/signature.

3.  Notary is not required!
